Primary & Community Care

Primary, Community and Continuing Care services are the point of first contact for people in the community that need to access health services. Internationally there is increasing recognition of the importance of the primary care system for improving health outcomes and managing costs. Stronger primary health systems are associated with better health outcomes and lower costs, particularly for children. Improved primary health and community support also has the potential to prevent hospital admissions, particularly for ambulatory sensitive conditions. As well, better primary health and community support can also prevent inappropriate and unnecessary use of residential care.

In order to maximise the quality and safety of this first line care there is a need for practical, integrated systems, which not only assist in providing effective care, but also ensures action when, regrettably, a significant incident occurs. HCI works with Primary and Community Care areas to maximise the care processes in use. An example of HCI work includes the Review of Clinical Governance Structures and Process Mapping in the Wexford Local Health Office.

With the introduction of regulation in this sector, particularly with regards to Primary Care, expected shortly, there is an increase need to ensure effective communication systems, defined and documented processes and ongoing audit. HCI provides all that is required.
